You must apply through a registry agent and are required to obtain electronic fingerprints for the Name Change application if you are 12 years of age or older

The document is required at times when you would like to change the name shown on certain identity documents such as passport and driver’s license. If you were born in Canada, a legal change of name also changes the name on your birth certificate.
