How to fill out the Chinese Embassy Authentication Form

*Please use black ink only for the content of the form.

Section 1:

• Fill in the applicant’s personal information.
• If the applicant is retired or unemployed, please write “retired” or “unemployed”.
• For Type of ID, please write Passport, and in Number of ID, write your passport number.
• For Home Address and Phone Number, please write your Chinese address and Chinese phone number.

Section 2:

• Please leave this section empty

Section 3:

• Please select the document you need to get authenticated. If you want to authenticate your criminal record check, please select “Certificate of non-criminal record”.

Section 4:

• Purpose of Legalization: please select according to your personal situation.
• For Destination of Legalization, please write the specific city in China you will be submitting the authenticated documents at. For example, if you will be submitting the documents in Shanghai, please write Shanghai.

Section 5:

• Please write Passport Copies next to Supporting Documents
• Next to Copies, please write 1

Section 6:

• Processing Time, please select Regular Service. The Chinese Embassy no longer offers express or rush service now due to COVID restrictions.

Section 7:

• Please leave this section empty

Section 8:

• Please handwrite your signature with black pen
• Date of the signature must be less than six months from the date of application.
